Flinders Chronic Condition Management Program™ Workshop

The Flinders Program™ chronic care philosophy and tools present an assessment, planning and motivational process which has been applied to chronic medical or mental health conditions and co-morbidities. The Flinders Program™ encourages health practitioners to support their patients to effectively manage their symptoms and the effects of their conditions over the long-term by adopting and maintaining behaviours that can delay disease development or progression.

The two-day face to face workshop will allow you to:
The workshops will be offered at set times throughout the year at WACRH in Geraldton. If sufficient participant numbers are enrolled, workshops will be offered at other rural sites by negotiation. Cost of the workshop may vary depending on the delivery site.
• Gain an understanding of the Flinders Program™ in chronic condition management and self-management
• Practice the assessment and care planning processes with a volunteer
• Receive a Certificate of Completion and license to use the Flinders Program™ in your practice
• Access further opportunities to submit additional care plans and achieve a Flinders University Certificate of Competence in Chronic Condition Management, with support from Flinders Program™ facilitator based at CUCRH
